Our Medicare patients have access to a team of experienced Medicare Coordinators available to answer questions, aid in problem solving, and communicate with providers.

They are available in the office, via phone, or email.

Medicare patients with chronic issues such as hypertension or diabetes may quality for our Remote Patient Monitoring and Chronic Care Management Programs.

Patients who meet program qualifications are required to enroll.

What is it?

RPM – Remote Patient Monitoring

RPM is remote patient monitoring, which analyzes trends in physiological measurements such as blood pressure, blood glucose, weight, and heart rate.

CCM – Chronic Care Management

CCM is for patients who have two or more chronic condition. CCM uses a comprehensive care plan that lists your health problems and goals.

 

BENEFITS

RPM

It is proactive management of your condition by reducing visits to the emergency department, hospitalizations, length of hospital stays, and possibly reducing office visits. Maintaining a healthy relationship with our team and office.

CCM

This program is a way that as health care providers, we can help you manage your chronic conditions and improve quality of life, thus reducing visits to the emergency department, hospitalizations, and length of hospital stays. Possible reduction of office visits as well as the remote monitoring program. Also, maintaining a health relationship with our office.

 

WHAT WE DO

RPM

Our office provides the device with a cellular chip, which allows us to obtain your data (for example, your blood pressure) when using the device.

CCM

One of our coordinators will contact you monthly, taking 10 to 15 minutes by phone to discuss your conditions, medications, lifestyle, etc.

 

WHAT WE NEED FROM YOU

RPM

We ask that the patient use the device at least once a day so that we can better monitor their health. As well as answering calls or responding to our text messages. Talk to our Medicare coordinator and let them know the best time and method (call/text/email) to reach you.

CCM

We ask you to answer calls or respond to any of our contact methods. Talk to our Medicare coordinator and let them know the best time and method (call/text/email) to reach you.

 

COST FOR YOU

The cost can vary from a copay to $0 dollars. We will  work with you if you cannot afford the copay. We believe that these programs improve your quality of life and demonstrate our commitment to medical care and service quality.
